Dr. Chase Madden, finally recuperating from torturous wounds inflicted by a psychopath, is persuaded by Juan to become involved in what he thought was a nuisance case; just something to take her mind off of the past. However, as murder, attempted murder and the kidnappings escalate, Juan has to redefine the case and Dr. Chase, not wanting to turn loose of such a good puzzle, is left wondering, "who is targeting identical twins from around the country and why?" Sheriff McGrath's resources are strained and he seeks help from the FBI. The British Foreign Office becomes involved at the insistence of the father of two of the victims. Juan disappears. Dr. Chase goes on the offensive. Sheriff McGrath knows stubborn when he's slapped with it and deputizes her as a way of keeping some control over the search for Juan and the kidnap victims. Mari's husband, Lord Bennington goes underground to investigate an IVF clinic and Dr. Chase enlists the unlikely help of a militia group to find Juan or his body and stumbles upon the kidnappers 'Safe House'. Can everyone work together to free the victims before the 'Safe House' is blown up?
